The Course Catalog Information Form (CCI), Course Catalog Topics Form (CCT) and Course Catalog One/First Time Offering Form (CCO) provide necessary information to UREG to populate the Course Catalog in the Student Information System (SIS). All schools have been asked to designate an “approved sender” who submits these forms for entry into the SIS Course Catalog. All forms are fillable Microsoft Word documents. On request a pdf version is available. Please fill out the form electronically, save the file, and e-mail it as an attachment to the next person in the approval process.

Course Catalog Information Form (CCI)

The purpose of this form is to create a new course, inactivate a course no longer offered, activate an inactive course, or change an existing course. There are checkboxes on the form to indicate the type of update that is being requested.

  • The “Effective Term” field, at the top left corner of the page, indicates the first instance the change requested will happen. For example, if the course title is changing, then this field will designate the first date this change will appear in the Schedule of Classes. In other words, SIS contains the functionality necessary to “future date” changes.
  • The “Grading Basis” restricts the grading option available for this course. The broadest choice for this field is “Student Option.” This allows the course to be either graded or offered as Credit/No Credit. Choosing “Graded” will preclude students from taking this course as Credit/No Credit.
  • The credit hours associated with a course must follow the guidelines provided in the Provost’s policy on Determination and Assignation of Academic Credit.
  • If the form is being used to create a new course, then the “Proposed” fields should be completed, as well as any other applicable fields.
  • To make a change to an existing course and, at a minimum, provide the School/College, Subject Area & Catalog Number, and Full title. In addition, the “Present” and “Proposed” fields should both be completed for any changes. In the Justification/Comments field you will need to mention the requested change.
  • To inactivate a course, provide the School/College, Subject Area & Catalog Number, and Full title.

Course Catalog Topics Form (CCT)

The purpose of this form is to create a course topic associated with a specific course. Topics may only be designated for courses numbered in the X500-X599 range in each thousand series.

  • The form may be used to add a topic that may be repeatedly taught.
  • All applicable fields should be completed.

Course Catalog One/First Time Offering Form (CCO)

The purpose of this form is to create a one time or first time offering. These courses can only be offered one term and may only be designated for courses numbered X559 in each thousand series.

  • The form may be used to add a topic as a first-time experimental course offering or to add a topic that may be offered one time.
  • All applicable fields should be completed.
